We’ve Done ‘Em All Our Lives, But Where DID These Halloween Traditions Come From?

It's called a jack-o'-lantern. But why "Jack?" Why not a Ted-o'-lantern? What about "trick or treat?" Isn't that an ultimatum? What about masks?
